All registered on-campus youth programs are required to report any incident (behavior/physical) and/or injury that occurs at camp or results from camping activities. 

When you complete the form online a copy of the report is automatically sent to you, Environmental Health and Safety and The Office Youth Programs at Â鶹´«Ã½.
